How to Make a Consultation

As specialists we would first require a referral from your normal dentist, GP or specialist.

If you have been told by your dentist or GP that you need to see one of our specialists please contact our reception team on 08 9328 3006 and they will be ready to help you arrange a convenient time.
Alternatively, your dentist or GP can complete the online referral form, or send a referral letter via any other secure communication pathways they prefer, and then one of our reception team will contact you to arrange a convenient time for your consultation.

For patients living far from Perth (more than 150 km), we will attempt to arrange your consultation and surgery on the same day or within 2-3 days. Please mention to the receptionist that you would prefer the "Country Patient Treatment" when booking your appointment. 
Please note that this may not always be possible, depending on the reason for your referral.

What to expect from your Initial Consulation

Your Initial consultation will be an opportunity to discuss your issues with your Surgeon and find out what type of surgery would be most suited to your needs.
In most cases, our Treatment Coordinators will then offer you a quote for surgery, and help you book a surgical date. We aim to book your surgery within a month of your consultation, but for some patients, we may need follow-up appointments for planning purposes or further test or scan before booking your surgery.

What you will need to bring with you to your consultation

  • Your completed New Patient Form and Medical History.
  • Your Medicare card - this is important for claiming purposes
  • Your Private Health Insurance details - this again is important for your claiming purposes
  • A list of medication you are currently taking - this is very important for treatment planning and if appropriate you may be asked to see one of consulting Anesthetists.
  • If you have been given your  referral or x-ray by your referring doctor or dentist please make sure that you bring this with you
